Gastro Esophageal Reflux Disease (GERD)
Gastroesophageal Reflux Disease (GERD) is a situation in which the stomach contents (food or liquid) leak backwards from the stomach into the esophagus (the tube from the mouth to the stomach). This act can rub the esophagus in the wrong way, causing heartburn and other symptoms. The esophagus is a long tube of muscle than runs from the mouth to the stomach.
Causes of Gastro ? Esophageal Reflux Disease (GERD)
Significant factor in GERD is the Lower Oesophageal Sphincter (LOS) muscle not working properly. The LOS acts like a valve, opening to let food fall into the stomach and closing to prevent acid leaking out of the stomach and into the esophagus. In cases of GERD, the LOS does not close properly, allowing acid to leak up out of the stomach.
Acid Reflux
Difference between Heart Burn and Acid Reflux
There is dissimilarity between Heartburn and Acid Reflux. Acid reflux is the action, while heartburn is the sensation. The pain is heartburn, whereas the movement of acid into the esophagus from the stomach is acid reflux.
- Acid reflux takes place when stomach acid reiterates up into the esophagus. Reflux can be one of the causes of heartburn. However, no pain at all may be felt when reflux occurs.
- Heartburn is a feeling of tightness, pain or discomfort in the center of the chest that can trail an occurrence of acid reflux or otherwise. Heartburn is exactly what it would feel like if acid ate away at the lining of your esophagus, because that?s what?s happening.

- Heartburn. A burning feeling in your chest (heartburn), sometimes extending to the throat, together with a sour taste in your mouth
- Asthma. ?Gastric juices ooze upwards into the throat, mouth and air passages of the lungs

- Dysphagia. Difficulty in swallowing

- Gastric H2 receptor blockers (such as ranitidine, famotidine and cimetidine) can reduce gastric secretion of acid. These drugs are technically antihistamines. They relieve complaints in about 50% of all GERD patients. Compared to placebo (which also is associated with symptom improvement), they have a number needed to treat (NNT) of eight (8).
- Antacids before meals or symptomatically after symptoms begin can reduce gastric acidity (increase pH).
- Alginic acid (Gaviscon) may coat the mucosa as well as increase pH and decrease reflux. A meta-analysis of randomized controlled trials suggests alginic acid may be the most effective of non-prescription treatments with a NNT of four.
- Prokinetics strengthen the lower esophageal sphincter (LES) and speed up gastric emptying. Cisapride, a member of this class, was withdrawn from the market for causing long QT syndrome. Reglan (metoclopramide) is a prokinetic with a better side-effect profile.
- Sucralfate (Carafate) is also useful as an adjunct in helping to heal and prevent esophageal damage caused by GERD, however it must be taken several times daily and at least two (2) hours apart from meals and medications.
- Mosapride citrate is a 5-HT4 receptor agonist largely as a therapy for GERD and dyspepsia.
- Baclofen is an agonist of GABAB receptor. In addition to its skeletal muscle relaxant properties, it has also shown to decrease transient lower esophageal sphincter relaxations at a dose of 10mg given four times daily. Reductions in esophageal relaxation clinically reduce episodes of reflux

Hot, dry weather heightens West Nile virus risk
CHICAGO (AP) ? Life's a picnic this year for the small, sneaky mosquitoes that carry West Nile virus and pose a deadly risk to humans. Hot, dry weather in the Midwest has created the perfect conditions in still-damp ditches and underground storm water basins where the Culex mosquito breeds.
Where there's water in the drought-plagued Midwest, it's stagnant water ? the Culex mosquito's favorite breeding habitat. The heat also speeds up the mosquito's life cycle, which means more breeding and more mosquitoes, and accelerates the West Nile virus replication process.
Illinois, Indiana, Ohio and Texas are reporting higher rates of infected mosquitoes compared with past years. More infected mosquitoes mean a higher risk for humans. Minnesota, Illinois, Oklahoma and Texas are reporting earlier-than-usual cases of human infection.
"I am quite concerned we may be facing one of our most severe seasons for West Nile virus since it arrived in our state in 2002," said Kristy Bradley, state epidemiologist in Oklahoma, which has had eight confirmed human cases of West Nile infection, with seven of those being the serious, neuroinvasive form of the disease. "I'm somewhat bracing myself for a rocky road ahead."
West Nile virus is a cyclical disease in humans, appearing in the summer and disappearing in the winter as carrier mosquitoes die off and hibernate.
Illinois, which usually sees its first human cases in August, already has two confirmed cases this year. Both are women in their 60s from the Chicago suburbs. Both were hospitalized and are receiving further care in rehabilitation centers.
"The risk is high and people need to listen," said Linn Haramis, an entomologist with the Illinois Department of Public Health. "This thing could put you in a wheelchair at age 60 for the rest of your life."
Most people infected with West Nile virus won't get sick, but approximately one in 150 people will develop the severe form of the illness. Symptoms include headache, high fever, neck stiffness, disorientation, coma, tremors, convulsions, muscle weakness and paralysis.
The best advice? Wear insect repellent containing DEET, picaridin or oil of lemon eucalyptus. Avoid being outdoors between dusk and dawn, when mosquitoes are biting. Install or repair screens to keep mosquitoes outside. Drain standing water to eliminate breeding habitat.
Haramis notes that in Illinois the rate of infected mosquitoes is the highest in a decade and higher than in 2002 when 67 people died. But he won't make predictions because a streak of cool weather or heavy rain could "knock the legs out of this thing."
Minnesota's first human case came in late May, much earlier than in previous years. The Minnesota man got sick with West Nile encephalitis and meningitis after traveling in the south-central part of the state. He's recovering after being hospitalized.
In Texas, 111 people have fallen ill with West Nile infection, more than double the 10-year average for cases reported before August. Most of the cases were serious and one person has died, according to Texas Department of State Health Services spokeswoman Christine Mann.
To be sure, some states ? Iowa, Wisconsin and South Dakota among them ? are seeing normal numbers of infected mosquitoes and no human cases.
"We're still warning cities that they have to do mosquito control," said Lon Kightlinger, state epidemiologist at the South Dakota Department of Health. "We don't want people to be fooled by the drought."
What's deceptive about the drought is the scarcity of pesky floodwater mosquitoes. The absence of the larger, bolder mosquito variety makes people think mosquitoes aren't a problem and gives the Culex mosquito a chance to sneak up and feed. Health officials are urging people to wear insect repellent even though they may not notice any biting mosquitoes.
Culex mosquitoes are secretive in their behavior, approaching quietly from behind.
"Before you know it, they've bitten you. And by that time, it's too late," said Bradley in Oklahoma.

James Holmes to Police: I'm The Joker
We have a disturbing update on James Holmes, the 24-year old suspected of killing 12 people and injuring dozens more inside an Aurora, Colorado movie theater this morning.
- The shooting took place at approximately 12:38 a.m. local time.
- Holmes protected himself with a ballistic helmet, a vest, a throat protector, a gas mask and black tactical gloves
- Officials believe Holmes acted on his own: “We are not looking for any other suspects,” said Oates. “We are confident he acted alone.”
- Holmes possessed three weapons when he was arrested behind the theater: an AR-15 assault rifle, a Remington shotgun and a 40-caliber Glock handgun. All were purchased legally.
- Holmes' hair was painted red and he told police, upon being apprehended, that he was "The Joker."
At last count, 71 people were wounded in this gunman's attack. President Obama addressed the nation earlier today, honoring the victims and asking America's to treasure the fragility of life.
Mitt Romney, meanwhile, has also made a speech in which he offered his "deepest condolences" to those affected by the tragedy and added:
"I stand before you today not as a man running for office but as a father and grandfather, a husband, an American. This is a time for each of us to look into our hearts and remember how much we love one another and how much we love and how much we care for our great country. There’s so much love and goodness in the heart of America."

China: Challenges to economic growth noted
BEIJING, July 18 (UPI) -- China's slowing economic growth is "an uncomfortable ebbing," for the country, the official Xinhua news agency said Thursday.
Citing the second quarter's 7.6 GDP growth, a Xinhua news analysis said it was sharply down from 9.5 percent a year earlier and half a percentage point below 8.1 percent in the first quarter.
"Although the pace is still enviable for economies elsewhere, it marks an uncomfortable ebbing for China," the report said. "It was the lowest rate since the 2008-2009 global recession, when China's growth eased to 6.6 percent in the first quarter 2009 before a massive government stimulus package provided double-digit growth."
While the slowdown is partly due to global economic woes such as the crisis in the Eurozone and slow recovery in the United States, China's largest trade partner, Xinhua said it is also self-induced, as the Chinese government has sought to tame high inflation and ease runaway real estate prices.
While the government's policies have largely worked, "these efforts have coincided with the global downturn and domestic economic rebalancing, sparking concerns that the slowdown might be deeper than initially expected," it said.
"The ripple effect of slowing growth is beginning to weigh on enterprises' profitability, exacerbating their financial stress amid rising costs and dwindling demand, as warnings of lower profits or even losses set to overshadow the market in the coming weeks," Xinhua said.
The report echoed Chinese Premier Wen Jiabao's warning this week that the economy has not formed a stable recovery and economic difficulties may continue for some time.
The report said analysts worry relying on the government to increase investment to spur growth will set back efforts to wean the economy off its reliance on state investment, lead to the construction of unneeded factories and saddle the economy with bad debts.

TORONTO (Reuters) - Canada's dollar rose against its U.S. counterpart on Wednesday, boosted by broad firmness in global equities and as the Bank of Canada kept its rate-hike stance while most other major advanced economies are moving in the opposite direction.
The currency rose to C$1.0101 versus the greenback, or 99.00 U.S. cents, its strongest level since July 5, as Governor Mark Carney defended its position by saying Canada cannot "cut and paste" monetary policy.
"We've weathered the storm a lot better than most other countries, especially in Europe and the States. The market is still taking his comments that rates will have to go up some time as a positive," said David Bradley, director of foreign exchange trading at Scotiabank.
" The Canadian dollar is benefiting from that purely from a yield play."
The Canadian dollar ended at C$1.0107 versus its U.S. counterpart, or 98.94 U.S. cents, up from Tuesday's finish at C$1.0126 or 98.76 U.S. cents.
Also supporting the currency was a move higher in global equities, with the S&P 500 <.SPX> hitting its highest level since early May on Wednesday as corporate profits from bellwethers like Intel <INTC.O> and Honeywell <HON.N> defied the market's fears about global growth. <.N>
Equities were also supported by optimism Federal Reserve Chairman Ben Bernanke may act to stimulate the U.S. economy if needed, underscoring his concerns especially in the job market.
But stocks diverged from other asset classes. The euro fell broadly on Wednesday after comments by German Chancellor Angela Merkel reignited worries about the euro zone debt crisis and government bond prices rose over fears of slow economic growth.

Germany - considered Europe's safest haven - sold bonds at a negative yield for the first time.
The top-up auction of two-year bonds raised 4.173 billion euros and follows short-term Treasury bill sales at negative yields by Germany and other higher-rated euro zone states.
Traders also cited a media report that quoted German Chancellor Angela Merkel as saying she could not be sure the European project would work.
"I think that Germany issuing two-year bonds with a negative yield for the first time and Merkel's comments certainly, in my mind, underscore what's wrong in Europe and will weigh on the euro," said Jack Spitz, managing director of foreign exchange at National Bank Financial.
Spitz noted that the 200-day moving average was still providing significant support for the U.S. dollar versus Canada's around C$1.0109: "A close below the 200-day moving average technically does set up dollar/Canada for a move lower towards better support near parity."
Canadian bond prices climbed across the curve with the two-year government bond up 2 Canadian cents to yield 0.964 percent, while the benchmark 10-year bond added gained 24 Canadian cents to yield at 1.616 percent.

Anderson never loses his core themes - young love, the need to escape, the bind and bluster of family. His "Kingdom" may not be large, but it is perfectly appointed.
Though undeniably smart and charming, "Moonrise Kingdom" loves itself the way the callow Holden Caulfield loves himself: unconditionally. Salinger understood the problem with that. Anderson may not.
The latest unadulterated delight from Wes Anderson, director of "Rushmore," "The Royal Tenenbaums" and "Fantastic Mr. Fox."
The usual complaints and caveats about Anderson - he's precious, his characters have no grounding in the real world - can be made about Moonrise Kingdom, but so what?
Anderson and his actors are able to convey more genuine feeling through these devices than most filmmakers can with more-traditional means.
One knock against some of Anderson's previous efforts is that they're too clever - so clever, in fact, that the humanity gets sucked out of them. That doesn't happen here.
Pure Joy. This film's heroes may be pint-sized but their emotions and dreams are larger than life.
My immediate response is that it's a masterpiece and the best film to come out so far this year.
The offbeat, pixilated universes created by writer/director Wes Anderson (Rushmore, The Royal Tenenbaums, Fantastic Mr. Fox) are an acquired taste.
It's hard to judge a movie as a movie when you want to live inside it.
Magical Moonrise a Romantic Delight.
Intermittently amusing, distinctive, not always easy to digest
Moonrise Kingdom left me thunderstruck. ... Anderson contrasts a world in disorder with a well-pitched campsite, where everything is in its right place.
Anderson's movies work best if they're about kids or cartoon animals.
Among the all-stars, Edward Norton made me repeatedly chuckle, and it's always a pleasure to see Bruce Willis when he's not operating in paycheck-whore mode.
A sweetly kooky ode to the intensity of young love. The movie balances broad comedy with occasionally surreal quirk, while still holding the emotional center.
The adults who are so desperate to return the kid fugitives to their care actually envy the children's freedom, irresponsibility and, yes, love -- in a word, their youth: The kingdom that makes exiles of us all.
The frustrating Anderson is improving, and he may one day shed the collegiate hipster aura that infuses his work.
The plot isn't as fetching as the presentation, starting with the tersely whimsical dialogue. There aren't punch lines for big laughs, but rather a seamless series of non sequiturs for smiles.
It's funny, it's touching, it's Anderson doing what he does better than he's ever done before.
Wonderfully inventive and entirely immersing.
The great thing about Wes Anderson, other than his fabulously childlike imagination, is his deep attention to the smallest details.
This isn't Wes Anderson's best effort by any stretch. However, any Anderson is better than no Anderson at all.
This film contains some of cinema history's best lightning strikes.

As homicides spike, Chicago mayor defends tactics
CHICAGO ?
Chicago's mayor and police superintendent publicly defended their new gang-fighting strategy Monday amid growing criticism that the changes are failing and a big reason why the city's homicide rate has soared this year.
After weeks of media reports about Chicago homicides - which so far are up nearly 38 percent from last year - Mayor Rahm Emanuel and Police Superintendent Garry McCarthy defiantly said during a news conference that the gang strategies in place before McCarthy arrived were the ones that failed, not the new ones.
More beat officers are now on the streets and staying in specific areas, replacing the large, specialized units that would temporarily drop into crime-ridden areas. Emanuel and McCarthy said they have no plans to change that strategy, and the mayor announced Monday that he's devoting another $4 million to tear down vacant buildings where gang members live and store guns and drugs.
The old tactic of flooding high-crime areas with teams of hundreds of officers for a short period of time, then moving the teams to other areas, was "like putting a band-aid on a gunshot wound," McCarthy said. "We're not repairing anything by doing that."
Emanuel, who has made attacking the city's street gangs a cornerstone of his administration, was equally dismissive of the effectiveness of those citywide units.
"I don't think coming in, swatting something down and letting it come back in two weeks is strengthening a community," he said. "What it does is build up cynicism."
However, the mayor and police leader rolled out their new anti-gang strategies at a time when the homicide rate was quickly increasing. Among the more recent high-profile deaths was a 7-year-old girl hit by a stray bullet while selling candy outside her home.
And on Monday, the Chicago Tribune ran a front-page story that included blistering criticism from two aldermen whose wards have been the center of much of the gang violence. One alderman said gang members no longer fear that they'll encounter the so-called strike units, which included dozens or even hundreds of officers who would temporarily saturate violent neighborhoods.
Chicago has averaged about 450 homicides a year since 2005, which is a dramatic drop from the roughly 900 homicides the city was experiencing annually in the early 1990s. Still, McCarthy said the current numbers are unacceptable - and noted that the city recorded about the same number of homicides last year as New York City, which is three times the size of Chicago.
"That's not success and I'm not willing to take it as success," said McCarthy, who spent much of his career as a New York City policeman.
He and Emanuel insisted that their crime-fighting strategies are paying off, noting that the city's overall crime rate has dropped 10 percent compared to the same time last year. McCarthy also said there is evidence that police are on the right track in their fight against street gangs.
"This weekend, there was half the number of shootings, half the number of murders as we had the same time last year," he said. He also noted that the amount of shootings over the weekend marked the fewest since February.
Quietly, though, some police officers have wondered about the wisdom of disbanding the large specialized units that were deployed by McCarthy's two immediate predecessors, Jody Weis and Phil Cline.
Weis said Monday that the units, which were sent to locations where a violent incident had occurred or in anticipation of where one might occur, were a big reason why the number of homicides fell when Cline was superintendent and during his last two years on the job.
"No one is proud of that, 450 homicides. But before these units were formed, there were 600," said Weis, a former FBI agent who was Chicago's police commissioner from 2008 until early 2011.
McCarthy, who replaced Weis when Emanuel became mayor, said that while Chicago's police force no longer has those citywide units, it finally has a citywide gang-fighting strategy that it never had before - one that pays special attention to the fact that much of the violence is tied to gangs retaliating against each other.
And Emanuel said there are other components of that citywide strategy, including targeting liquor stores and other businesses that police and community activists have identified as gang hangouts. Several have been closed.
Police also are doing gang audits, so they can identify where specific gangs operate and their members. And the plan the mayor announced Monday calls for the city to spend $4 million to tear down or board up vacant buildings that have been identified as places where gangs live, deal and store drugs, and hide guns.
Gangs, the mayor said, "will not find shelter in the city of Chicago."
